A Study of Degeneration of the Dorsal Vagal Nucleus in Parkinson¡¯s disease and Lewy body Disease


Abstract
In Parkinson¡¯s disease several subcortical nuclei, such as substantia nigra, locus ceruleus, hypothalamic nuclei, basal nucleus of Meynert etc., are involved. However, there has not been investigated about the difference between idiopathic Parkinson¡¯s disease(Lewy body positive parkinson¡¯s disease) and secondary parkinsonism (non-Lewy body parkinsonian symdrome). The author designed this study to investigated the pattern of the degeneration of the locus ceruleus and dorsal vagal nucleus in idiopathic Parkinson¡¯s disease, non-Lewy body parkinsoniam syndrome and incidental Lewy body disease without evident parkinsonian symptoms. The results were as follow; 1) Male to female ratio was 2:1 in Lewy body negative parkinsonism and 4.7:1 in Lewy body positive parkinsonism(idiopathic Parkinson¡¯s disease), however, it was 1:1 in Lewy body disease without evident parkinsonism, 2) Degeneration in the locus ceruleus and dorsal vagal nucleus were hardly found in Lewy body negative parkinsonism, even though 93.1% of patient with Lewy body positive Parkinson¡¯s disease, showed degeneration in locus cerulues and 71.6% of them showed degeneration in dorsal vagal nucleus, 3) Degeneration of the locus ceruleus were found in the patient with Lewy body disease regardless they had parkinsonism. However, degeneration of the dorsal vagal nucleus were frequently found in the cases with parkinsonism than without parkinsonism. With the result of this study, it is difficult to use the degeneration of the locus ceruleus and dorsal vagal nucleus as a pathological marker of Lewy body-positive Parkinson¡¯s disease. However, the author suggest that our findings were supportive to that Lewy body disease without parkinsonism are preclilnical stage of idiopathic Parkinson¡¯s disease. ferent pathogenesis, because degeneration of the locus ceruleus and dorsal vagal nucleus were noted in Lewy body negative parkinsonism.
